Making a small lip bigger is, far and away, the most common cosmetic lip change requested. However, there are some patients, both men and women, that feel their lips are too big and desire that they be smaller. When one’s lips are naturally excessively large, they may functionally interfere with lip competence (coming together and creating a lip seal) as well as being a source of too much attention and embarrassment.

Very large lips, also known as macrocheilia, can develop for multiple reasons. Some people just have them naturally and others develop them from multiple uncommon medical reasons. While large lips have a significant ethnic association, I have also seen a number of Caucasians from various descents who have them as well. In my Indianapolis plastic surgery practice, I have treated as many Caucasians as other ethnic groups for lip reduction surgery.

One lip condition that can be confused with macrocheilia is the double lip which occurs only in the upper lip. This is because the upper lip is too large (too much outer vermilion), it is because there is an extra roll of tissue on the underside (mucosa) of the upper lip. This becomes apparent when one smiles, as this extra roll of mucosa is seen as a horizontal roll of tissue beneath the upper lip. It surgical removal is done slightly different than a large outer upper lip.

The objective of lip reduction surgery is to create lip sizes that are more proportionate to each other as well as the rest of the face. At the least, a patient’s goal is to not have them as the most dominant feature of one’s face that draws the eye right to them.

Lip reduction surgery is uncomplicated to do but the surgical planning (marking) is the key. An initial mark (line) is drawn at the wet-dry junction but may change based on which part of the lip is more prominent, the dry vermilion or the wet mucosa. For some large lips, the dry vermilion is the dominant part to remove. For others, it can be more of the wet vermilion and mucosa. Careful preoperative examination of the lip must be done to make this determination. No matter which part of the upper lip is removed, you do not want the final closure (and subsequent scar) to be evident when the lips are either in rest or smiling positions. A wedge of excess lip is marked out and tapered as it goes into the corners of the mouth to prevent ant tissue excesses. I like to keep the final edge of the cut out away a few millimeters from the corner of the mouth.

Lip reductions can be done under local anesthesia, although more significant anesthesia would be more pleasant. Once anesthetized, the wedge excision removes primarily outer mucosa and the underneath submucosa. (in medical macrocheila the lip tissues removed may be deeper and more extensive) It is not necessary to remove muscle for cosmetic reduction and the labial artery should be preserved. I find that a pinch technique is very useful. By this method, the planned amount of mucosal excision is pinched up with a special instrument until the entire desired amount is in the pinch. This is then cut off with scissors and closed with a resorbable running suture. Only antibiotic ointment or vaseline is applied to the dry vermilion after to keep it moisturized as the lips swells and is prone to cracking and soreness.

The lip will swell considerably, which is normal, and it may taken several weeks to appreciate the amount of reduction that has been done. Usually, about 4 to 6mm reduction sometimes more) of actual horizontal lip size can be easily achieved.

Lip Reduction Risks, Dangers and Complications

All surgeries carry with them an inherit possibility of having some type of complication or risk associated with them. Since the lip reduction Surgery is a surgery, although it may be considered minimally invasive, it is still a surgical Surgery and therefore carries its own list of potential risks and complications associated with it.

There are several possible complications associated with the surgery. For starters, there will be some form of anesthetic utilized during the Surgery and as with any type of anesthesia there is the chance of having an allergic reaction to that anesthetic. Furthermore, there may be the chance of having a hematoma, which is just an advanced way of saying that there might be some bleeding associated with the Surgery. There is the chance of having a seroma which once again is just a fancy term which means that there may be fluid build up in the lips after undergoing a lip reduction surgery. Of course there is the chance of having numbness in the lips and general mouth region as a result of the Surgery but this numbness is only temporary and will be gone by the end of the 7th day following a lip reduction surgery.

As with any plastic or cosmetic surgical Surgery there is always the possibility of asymmetry. What this means is that after undergoing the lip reduction Surgery it is completely possible for one side of the lips to look different than the other, and thus they are no longer symmetrical or the same. The lip reduction surgery can also affect the way that your smile looks if not for any other reason but the fact that a portion of the previous mass of the lips has been removed.

Plastic Surgery of the Lips – Making One’s Lips Smaller

In today’s society, large and full lips are highly desirous. This is most evident in the large plethora of lipstick and lip care products as well as being one of the top places on the face for the use of injectable fillers. Despite the trend to go bigger, there are a few patients who actually want their lips to be smaller. Most commonly, this are non-caucasian patients who naturally have very full lips or someone who has a rare congential facial problem which results in large lips, medically known as macrocheilia.

Lip reduction surgery is a fairly simple Surgery that can be done under local anesthesia or some form of sedation. It involves a transverse elliptical excision of lip tissue that runs from one corner of the mouth to the other. The key is the location of the excised tissue and the resultant scar. The exposed portion of your lip, the pink portion, is called the vermilion. The vermilion has both a dry and wet surface. The wet portion of the vermilion is the non-exposed part that sits just below the highest point of the dry vermilion (lip line) on the inside of the mouth. Removing excess vermilion here and allowing the lip to roll back and look smaller places the fine line scar in a non-visible location.

Reducing the size of the lips through this inside the mouth approach produces a modest size reduction, never a dramatic one. If one removes too much from the inside, the lips can have an unnatural rolled back appearance. In reality, the best place to reduction the visible size of the lips is by removing vermilion at the front edge where it meets skin. But this would create a visible scar which is why it is not done even though it is more effective at creating lip reduction.

Any form of lip surgery will create considerable swelling in the first few days after surgery. This is normal as the lip swells easily. (just think of when you just bumped it and see how quickly it swells!) As a general rule, the lip swelling subsides at a week out and the lip will look the same then as before surgery. You won’t be able to see visible lip size reduction until several weeks after surgery. So one has to be patient to see the results. Dissolveable stitches are used to close the incision on the lip as this is not a very nice place to have stitches that need to be removed later.

Lip Lifts, Augmentation, and Reduction

If someone wants fuller pouty lips, there are several options. One is cosmetic surgery. Procedures such as lip lifts and lip augmentation can give a man or woman larger lips. This Surgery can be done on either the upper or lower lip. Lip reduction goes the opposite way – it can make the lips smaller. But like any cosmetic surgery, there are limitations and risks, so make sure you get the facts first.

Lip lifts are done to create the impression of a fuller upper lip. Basically, an incision is made under the nose, and then the skin is pulled up. This basically winds up revealing more of the underside of the lip, which makes it look fuller. If someone wants even more of an increase in lip size, augmentation or enhancement can be done, using implants. The implants can be man-made or human tissue. Injectable lip fillers such as Restylane, collagen or Juvederm are another option. Injectable lip fillers are materials that are injected into the lips. They plump them up and last for several months. Some fillers are made of different materials and last over a year. Fat can be used to augment the lips as well.

For someone who thinks their lips are too large, lip reduction surgery may be an option. After an incision is made, excess tissue is removed. This surgery can be done on the upper or lower lip.

Lip surgery has risks, including infection, bruising and poor outcome. Since the lips are readily visible, anyone considering this wants to make sure to use a qualified board-certified plastic surgeon. Cost is also an issue, with costs ranging from $1500 to $3000. This is not going to be covered by insurance, so you would need to find another way to finance it.
